What happened
Insulet Corporation (NASDAQ: PODD) announced a buyback of up to $1 billion and raised its full-year margin guidance alongside third-quarter results. Sales grew faster and the operating margin widened, the pairing OptimistFi's thesis on Insulet needs.
Revenue rose 19.2% to $842.0 million from $706.3 million in the third quarter of 2025. In constant currency, growth was 19.6%, slightly above the high end of the company's 17.5% to 19.5% guidance range.
The board authorized the repurchase on Oct. 30, and the program runs through Dec. 31, 2028. It adds to earlier authorizations. Insulet may buy on the open market, in private deals or through an accelerated program. It does not have to buy any shares and can extend, change, suspend or end the program at any time.
Chief Executive Ashley McEvoy said the repurchase shows the company's confidence in the Omnipod platform and Insulet's long-term trajectory.

Where the growth came from
International sales did the heavy lifting. International Omnipod revenue grew 31.7% to $266.2 million, more than twice the 14.7% growth in U.S. Omnipod revenue, which reached $570.4 million. In constant currency, international growth was 33.0%, because currency took 1.3 percentage points off the reported figure.
Total Omnipod revenue was $836.6 million, up 19.7%, or 20.0% in constant currency. Drug Delivery revenue was $5.5 million, down 22.7% from $7.1 million a year earlier.
Insulet said it finished its U.S. sales force expansion and carried out a Type 2 action plan. The plan covers onboarding, customer support, sampling and sales pay tied to new customer starts and retention. It also finished the full U.S. release of Omnipod Discover. The company says the app is driving adoption by more than 35,000 global users and 5,000 healthcare professionals as of Nov. 2.

Margins and one-time items
Operating income was $189.8 million, or 22.5% of revenue, up 590 basis points from the third quarter of 2025. Adjusted operating income was $185.2 million, or 22.0% of revenue, up 490 basis points.
Two items separate the reported and adjusted figures. Adjusted results leave out a $4.5 million net adjustment to estimated warranty and related costs from voluntary device corrections issued in March and May 2026. They also leave out a $12.5 million gain in the year-earlier quarter on a derivative asset tied to the redemption of convertible notes.
Net income was $141.1 million, or $2.03 per diluted share, against $87.6 million, or $1.24, a year earlier. Adjusted net income of $138.0 million rose 57.2%, and adjusted earnings of $1.99 per diluted share rose 59.9%.
The new outlook and the buyback cash
Insulet narrowed its 2026 revenue outlook and lifted the low end. It now expects total constant-currency revenue growth of 21% to 22%, against 20% to 22% in its Aug. 5 guidance. Total Omnipod guidance is 21.5% to 22.5%, from 21% to 23%, and International Omnipod is 32% to 33%, from 30% to 32%. U.S. Omnipod is 17.5% to 18.5%, from 17% to 19%.
Adjusted operating margin is now guided to expand 150 basis points from 2025, up from 100 basis points. Adjusted EPS growth guidance stays at 30%.
Insulet plans to use about $400 million of the program for fourth-quarter repurchases, paid from existing cash. Cash and equivalents were $584.4 million on Sept. 30, down from $716.1 million at Dec. 31, 2025.
Why it matters
For holders, the quarter supports the growth and margin half of the case OptimistFi has made for Insulet: a recurring-consumable pod platform where manufacturing scale turns growth into higher margins and cash. Revenue and margins moved the right way. Cash has not kept pace.
Free cash flow for the first nine months was $211.7 million, down from $329.5 million a year earlier, a 35.8% decline by OptimistFi's calculation. Operating cash flow fell to $325.7 million from $386.0 million, while capital spending more than doubled to $113.9 million from $56.5 million.
Accounts receivable rose to $689.0 million from $516.9 million at Dec. 31, 2025, and inventories rose to $552.1 million from $452.6 million. The nine-month figures do not yet show growth turning into cash. The company also lists its reliance on one main product platform and its concentrated manufacturing among its risks.
What's next
Insulet's next quarterly report, for the period ending Dec. 31, 2026, will show how much of the planned fourth-quarter repurchase was done. It will also show whether the guided margin expansion and adjusted EPS growth held for the year.
Stronger free cash flow alongside actual buybacks would support the case. Another drop in free cash flow while cash falls would weaken it.
